The recognition of primary membranous nephropathy (pMN) as an autoantibody-driven disease and the identification of podocyte target antigens have been major advances in the diagnosis, prognosis, and management of pMN and support the use of B-cell depleting agents such as rituximab. However, many patients do not respond to rituximab, and others who respond experience relapse. Second-generation anti-CD20 agents and other B-cell targeting agents are in development for pMN. This expert discussion will highlight the need for and how emerging B-cell targeted therapies may be incorporated into the treatment of pMN.
