AKHOMM: Interdisciplinary approach to enhance goal-directed medication therapy in CKM: Strategies to manage hyperkalemia

This module will help interprofessional teams optimally treat cardio-kidney-metabolic (CKD) syndrome patients focusing on both the real risk and potential risk of hyperkalemia. With new KDIGO guidelines, we will focus on goal directed medication therapy in CKM and strategies to manage hyperkalemia. Objectives: By the end of the this module on CKM learners will be able to: Broadly define Cardiovascular-Kidney-Metabolic (CKM) syndrome and identify the key opportunities to optimize goal-directed medication therapy (GDMT). Describe epidemiology of hyperkalemia among people with CKM and highlight the frequency of GDMT dose reduction and discontinuation that impacts clinical outcomes and mortality. Identify non-pharmacologic and pharmacologic approaches to manage hyperkalemia and review data regarding proactive serum potassium management and continuation of GDMTs. Integrate patient risk factors, access and therapeutic goals in case-based examples that optimize GDMT in conjunction with effective serum potassium management strategies
