Identify the range of conditions and patients across the continuum that are served by certified home health agencies, and for which referral for services may be indicated. 3. Summarize the criteria, documentation, and processes for patient referral to certified home health agency services. 4. Examine the requirements for eligibility/qualification for referral, review/signature of orders and face-to-face certifications, calls from home health professionals.
