Communication Skills for Serious Illness Conversations: Course 3 of 5 Course Description: Learn best practices for building trust, eliciting patient values, and having patient-centered conversations about goals of care. Learning Outcomes: 1. Explain the foundation for discussions about future care. 2. Discuss how to use the five steps of the REMAP model for guiding conversations on care goals. 3. Describe an approach to introducing hospice to patients and families or caregivers when appropriate.
