Community Pulmonology Diagnostic Competition: A Simulation-Based Assessment in Fibrotic Interstitial Lung Disease

Identify the clinical and radiographic features (older age, progressive exertional dyspnea, persistent bibasilar inspiratory Velcro crackles, peripheral reticulation, traction bronchiectasis) that warrant prompt high-resolution CT and interstitial lung disease evaluation rather than continued empiric management of COPD or recurrent infection. Classify fibrotic ILD cases using the 2022 ATS/ERS/JRS/ALAT high-resolution CT pattern categories and determine the appropriate next diagnostic action, including referral for multidisciplinary discussion when the pattern is indeterminate or an alternative diagnosis is suspected. Differentiate IPF from fibrotic mimickers (fibrotic hypersensitivity pneumonitis, connective-tissue-disease associated ILD, fibrotic NSIP) using exposure history, serologic evaluation, and imaging features, and identify patients with IPF or progressive pulmonary fibrosis for whom antifibrotic therapy is indicated.
