Cranial Nerve III Palsy (Oculomotor Palsy)

Cranial nerve III palsy or oculomotor palsy results from dysfunction of the oculomotor nerve, producing ophthalmoplegia, ptosis, and variable pupillary involvement. Common etiologies include microvascular ischemia; compressive lesions, such as aneurysms and neoplasms; traumatic injury; hemorrhage; congenital anomalies; and idiopathic origins. Risk factors include hypertension, diabetes mellitus, atherosclerotic disease, and intracranial aneurysms. Pathophysiology varies according to cause, with ischemic injury typically sparing parasympathetic fibers and compressive lesions often causing pupillary dilation. Clinical presentation may feature diplopia, eyelid droop, strabismus, and impaired extraocular movements, with pupillary involvement serving as a key diagnostic discriminator. Evaluation requires a comprehensive neurologic and ophthalmologic examination, neuroimaging using magnetic resonance imaging or computed tomography angiography, and laboratory assessment for vascular or inflammatory disorders. Management is directed by the underlying cause and may include vascular risk optimization, surgical decompression, aneurysm repair, or supportive interventions such as prism lenses or patching. Potential complications include persistent diplopia, amblyopia in children, and vision-threatening compressive lesions. Prognosis depends on etiology, severity, and timeliness of intervention. This activity for healthcare professionals is designed to enhance learners’ competence in evaluating and managing oculomotor palsy. Participants advance their mastery of the condition’s relevant anatomy, etiology, risk factors, pathophysiology, clinical presentation, and evidence-based diagnostic and therapeutic approaches. Improved skills equip clinicians to collaborate with interprofessional teams providing care for individuals with this neurologic disorder.
