Diabetes Patient Education ECHO Program

1. The Diabetes Patient Education ECHO program is a virtual, case-based learning program designed to strengthen the knowledge, confidence, and practical skills of professionals delivering Diabetes Self-Management Education and Support (DSMES) and the National Diabetes Prevention Program (National DPP). Utilizing the Project ECHO model, this monthly series will connect with subject matter experts through interactive presentations, participant case discussions, and collaborative problem-solving. The program is intended to build workforce capacity by equipping participants with evidence-based strategies to improve diabetes prevention and management services, strengthen program sustainability, and increase access to high-quality education for individuals living with or at risk for diabetes. Sessions will emphasize practical implementation strategies that participants can immediately apply within their organizations.
