A wide range of pathologies can originate within or adjacent to the sphenoid sinus. Benign entities such as mycetomas, mucoceles, meningo-encephaloceles, fibrous dysplasia and pituitary lesions, or malignant tumours such as melanoma, chordoma or chondrosarcoma can present to the ENT surgeon with generalised, specific rhinological and sometimes neurological symptoms. The incidence of occult findings on cross sectional intracranial imaging is also rising, with a consequential increase in referrals. The appropriate diagnosis relies on a critical understanding of the anatomy of the sphenoid sinus and it surrounding structures that include the pituitary fossa, cavernous sinus and posterior fossa, as well as recognising the structures at greatest risk during the surgical treatment including the carotid artery, optic nerves and anteroposterior skull base. In this international symposium we aim to provide delegates with an up-to-date summary of the anatomy, pathology and surgical management of common and rare sphenoid sinus lesions using structured case based learning and panel discussion. We aim to provide a structured and practical treatment paradigm for general otolaryngologists and rhinologists.
