Gastrointestinal Neuroendocrine Tumors

Gastrointestinal neuroendocrine tumors (GI-NETs) are a heterogeneous group of neoplasms originating from neuroendocrine cells within the digestive tract, most commonly in the midgut, particularly the ileum and appendix. GI-NETs can be categorized based on their site of origin, hormonal activity, and tumor grade. These tumors generally exhibit slow growth, though their clinical behavior varies based on differentiation status, histological grade, and proliferation indices, with their aggressiveness ranging from indolent, well-differentiated tumors to aggressive neuroendocrine carcinomas (NECs). Poorly differentiated NECs demonstrate aggressive progression and poor outcomes. Some GI-NETs secrete bioactive hormones, leading to specific syndromes, eg, carcinoid syndrome, especially in cases with significant liver metastasis. Diagnosis relies on imaging modalities alongside biochemical markers. However, due to their usually indolent nature and nonspecific symptoms, GI-NETs are frequently diagnosed late, with metastases present in many cases. The specific characteristics of each tumor influence its management. Management approaches may include nonsurgical and surgical therapies, though surgical resection remains the primary treatment when feasible.
