Konno-Rastan Aortic Root Enlargement

The Konno-Rastan procedure is used to enlarge the aortic root and subvalvular region, primarily in cases of aortic stenosis and complex left ventricular outflow tract obstruction. This condition often presents in those with pediatric or congenital heart disease where standard valve replacement is not feasible due to a small or hypoplastic aortic annulus. By making a longitudinal incision through the right coronary sinus and extending it into the right ventricular outflow tract and the muscular septum, the procedure effectively increases the diameter of the aortic root and alleviates the obstruction. This significantly reduces transvalvular pressure gradient and promotes favorable left ventricular remodeling, improving long-term cardiac function and overall outcomes. Through this educational activity, participants gain detailed knowledge of the Konno-Rastan procedure, including its indications, surgical steps, equipment, and potential complications. Learners enhance their ability to plan and execute complex aortic root enlargements and understand when adjunct procedures may be necessary. Emphasis is placed on interprofessional collaboration among cardiothoracic surgeons, anesthesiologists, perfusionists, cardiologists, nurses, and operating room staff to optimize surgical outcomes, ensure patient safety, and facilitate comprehensive perioperative care.
