Level 4 – Behavior Change Theories Made Easy

Apply current standards of care and evidence-based guidelines to develop and adjust individualized treatment plans in complex diabetes cases. Integrate person-centered communication, trauma-informed care, and behavior change strategies, including motivational interviewing, to address barriers to self-care and support individuals managing diabetes in complex psychosocial and medical situations. Calculate and fine-tune insulin dosing using advanced pattern management strategies in patients with fluctuating needs, including those using pumps or those hospitalized. Conduct comprehensive assessments, such as foot risk evaluations and social determinants of health, to inform prevention and care planning in high-risk populations.
