PARP-Inhibitor Combination Treatments: Biomarker Testing, Safety, and Efficacy in a Rapidly Changing Metastatic Prostate Cancer (mPC) Landscape

Advanced prostate cancer remains a leading cause of cancer death among men. As the therapeutic landscape evolved to include combination therapies for gene mutated disease, with response to PARP inhibitors hinging on detecting germline or somatic DNA-repair defects, professional societies endorse universal germline testing plus broad next-generation sequencing once metastatic disease is diagnosed. In this video-based activity, leading experts explore germline and somatic testing indications, panels, and workflows that detect DDR/HRR alterations relevant to PARP-inhibitor use. This activity also addresses evidence for sequencing PARP inhibitors with ARPIs, taxanes, radioligand therapy, and immunotherapy in mHSPC and mCRPC and offers strategies for evaluating patient- and disease-specific factors when considering prostate-directed radiotherapy or cytoreductive surgery alongside systemic PARP-based regimens. This activity also focuses on shared-decision frameworks that incorporate patient-reported outcomes and tips for overcoming financial toxicity and other barriers to optimal outcomes.
