Team Documentation: Improve Efficiency of EHR Documentation

Electronic health records (EHRs) have changed the documentation process in physician practices. Tasks such as data gathering and data entry that were once handled by team members (eg, receptionists, medical transcriptionists, medical assistants [MAs], pharmacists, and nurses) have shifted to physicians. Time spent on these tasks detracts from time physicians can devote to their patients. Implementing team documentation can enhance documentation efficiency, reduce redundancy, provide better care to patients, and ease physician burnout.
