Pulmonary embolism (PE) is the most serious complication of venous thromboembolism (VTE) and remains the third leading cause of cardiovascular death in the United States, with hundreds of thousands of cases each year. Most PE-related deaths result not from treatment failure but from missed prevention opportunities or diagnostic error. Because PE frequently mimics other conditions such as pneumonia, COPD exacerbation, or syncope it remains a leading cause of patient harm and malpractice claims. Improving recognition of atypical presentations, applying appropriate diagnostic pathways, and clearly documenting clinical reasoning are critical to improving safety and reducing risk. This activity the second in a two-part series focuses on reducing diagnostic error in PE by addressing common diagnostic pitfalls, strategies for distinguishing PE from its mimics, and best practices for documentation. After completing this activity, practitioners and the healthcare team should be able to: integrate awareness of the frequency and causes of missed PE diagnoses into clinical practice to improve patient safety and reduce malpractice risk; differentiate PE from common mimicking conditions through careful clinical assessment and appropriate diagnostic techniques; and apply structured documentation practices that incorporate key clinical positives and negatives, support a defensible differential diagnosis, and promote high-quality care.
