Bacterial Endophthalmitis

Bacterial endophthalmitis is a vision-threatening intraocular infection that can lead to irreversible blindness without prompt recognition and treatment. The condition occurs following disruption of ocular integrity (eg, surgery or trauma) or via hematogenous spread. Notably, bacteremia or fungemia may be transient, resulting in presentations without systemic infection symptoms. Advanced molecular diagnostic methodologies, currently employed in research settings, facilitate expedited pathogen identification when conventional culture methods yield negative results. Intravitreal antibiotics are the cornerstone of treatment, with vitrectomy indicated in selected cases. Prognosis varies by causative organism and disease severity. Given this condition’s potential for rapid progression, endophthalmitis is an ophthalmologic emergency requiring immediate intervention. This course outlines the evaluation and management of bacterial endophthalmitis and reviews the interprofessional team’s role in coordinating care to optimize outcomes.
