• Providers will regularly utilize the MAT Alternative Agent Request Power Form to request an alternative agent for their patients as appropriate. • Providers will be aware of the changes to the X-waiver requirement, and medical leaders will be aware of methods to continue tracking MAT burden per Care Team. • Health care staff will be aware of the health care outcomes and benefits of the ISUDT program, and aware of future planned expansions and enhancements to the program. • Providers will be able to access and use the updated MAT Power Form and Addiction Medicine note template.
