Respiratory viruses are the most common cause of pharyngitis; however, bacterial etiologies, particularly Group A Streptococcal pharyngitis, remain clinically significant due to the risk of complications such as rheumatic fever and glomerulonephritis. Overlapping clinical features between viral and bacterial causes make accurate diagnosis challenging and necessitate appropriate use of diagnostic testing, including rapid antigen detection, nucleic acid amplification tests, or throat culture. This activity reviews evidence-based approaches to differentiating viral from bacterial pharyngitis, the appropriate application of clinical tools such as Centor criteria, and the recognition of less common bacterial pathogens. Participants will improve diagnostic accuracy, optimize antibiotic stewardship, and enhance clinical decision-making to reduce unnecessary testing and treatment while improving patient outcomes through effective, team-based care.
