Alternobaric Vertigo

Alternobaric vertigo is a benign vestibular condition caused by differential middle ear pressurization during changes in ambient pressure, most commonly encountered in scuba diving and aviation. This pressure mismatch produces asymmetric vestibular input, resulting in transient vertigo that typically resolves with pressure equalization. Although generally self-limited, symptoms may impair safety in high-risk environments and may mimic more serious conditions, such as inner ear barotrauma or decompression illness, making accurate recognition essential. Risk factors include eustachian tube dysfunction, recent upper respiratory infections, and prior barotrauma. This activity provides a focused review of the pathophysiology, clinical presentation, and diagnostic evaluation of alternobaric vertigo. Clinicians gain improved ability to distinguish benign from emergent causes of vertigo, apply appropriate examination techniques, and implement evidence-based management strategies, including patient education on equalization maneuvers. Emphasis is placed on risk stratification, safety considerations, and coordinated interprofessional care to optimize outcomes and prevent complications.
