Anterior Cord Syndrome (Ventral Cord Syndrome)

Anterior cord syndrome, also known as ventral cord syndrome or anterior spinal cord syndrome, is a rare, time-sensitive myelopathy characterized by bilateral motor deficits and loss of pain and temperature sensation with relative preservation of vibration and proprioception. The condition most commonly results from ischemia of the anterior spinal artery but may also arise from traumatic, compressive, perioperative, or hypotensive injury. Accurate differentiation from transverse myelitis, demyelinating disease, and compressive myelopathy is essential because early recognition and appropriate management may influence neurologic recovery. Magnetic resonance imaging with diffusion-weighted sequences and vascular assessment supports diagnosis, while treatment focuses on maintaining spinal cord perfusion, addressing the underlying cause, preventing secondary complications, and initiating rehabilitation. This activity reviews the clinical presentation, diagnostic evaluation, management, prognosis, and complications of anterior cord syndrome and emphasizes coordinated care among physicians, nurses, radiologists, rehabilitation specialists, and other healthcare professionals to optimize functional outcomes and quality of life.
