Arytenoid Subluxation

Arytenoid subluxation is an uncommon laryngeal injury characterized by partial displacement of the arytenoid cartilage within the cricoarytenoid joint. Complete displacement of the arytenoid cartilage is classified as arytenoid dislocation, a distinct clinical entity with separate diagnostic considerations. Arytenoid subluxation most frequently arises following airway instrumentation, including endotracheal intubation, or direct blunt or penetrating trauma to the larynx. Risk factors include prolonged or difficult intubation, high ventilatory pressures, congenital joint laxity, and preexisting laryngeal pathology. Displacement of the arytenoid cartilage disrupts normal vocal fold mobility, leading to impaired glottic closure and altered phonation. Patients typically present with persistent postoperative hoarseness, dysphagia, odynophonia, and, in severe cases, airway compromise. Diagnosis relies on laryngoscopic examination, often supplemented by flexible endoscopy or imaging studies to assess joint alignment. Management ranges from conservative voice therapy in mild cases to surgical reduction or arytenoid repositioning in significant or persistent dysfunction. Complications include chronic dysphonia, aspiration, and airway obstruction. Prognosis depends on the timeliness of intervention, with early recognition associated with improved functional recovery. This activity for healthcare professionals is designed to strengthen learners proficiency in evaluating and managing arytenoid subluxation. Participants will refine their grasp of the condition’s etiology, risk factors, pathophysiology, clinical presentation, and current diagnostic and therapeutic recommendations. Improved skills will enable clinicians to participate effectively in interprofessional care and optimize functional recovery for affected individuals.
