Asymptomatic Bacteriuria

Asymptomatic bacteriuria is defined as the presence of bacteria in a properly collected urine specimen without signs or symptoms of a urinary tract infection, and it is particularly common among older adults. Although most individuals experience no adverse outcomes and do not require antimicrobial therapy, treatment is indicated for selected populations, including pregnant patients, individuals undergoing urologic procedures associated with anticipated mucosal bleeding, and certain renal transplant recipients. This course reviews the epidemiology, diagnostic criteria, clinical evaluation, and evidence-based management of asymptomatic bacteriuria, with an emphasis on appropriate patient selection for treatment and antimicrobial stewardship. Participants learn to distinguish asymptomatic bacteriuria from symptomatic urinary tract infection, recognize appropriate indications for therapy, and avoid unnecessary antibiotic use. Collaboration among clinicians, infectious disease specialists, urologists, obstetric clinicians, pharmacists, nursing professionals, and other interprofessional team members promotes evidence-based care, reduces inappropriate antimicrobial prescribing, and improves patient outcomes.
