Best Disease

Best disease, also known as Best vitelliform macular dystrophy (BVMD), is a rare autosomal dominant inherited retinal disorder caused primarily by pathogenic variants in the BEST1 gene and characterized by incomplete penetrance and variable clinical expression. The condition typically presents during childhood and may lead to progressive central visual impairment through characteristic vitelliform macular lesions and associated retinal changes. This course reviews the genetics, pathophysiology, clinical presentation, diagnostic evaluation, disease staging, and evidence-based management of Best disease. Participants learn to recognize characteristic funduscopic and multimodal imaging findings, interpret electrophysiologic and genetic testing, differentiate Best disease from other inherited and acquired macular disorders, and develop appropriate monitoring and management strategies. Collaboration with an interprofessional healthcare team, including ophthalmology, genetics, low-vision rehabilitation, and primary care, enhances diagnostic accuracy, facilitates genetic counseling, supports longitudinal care, and optimizes visual and patient-centered outcomes.
