Blueprint in Practice: The Inherited Regimen – Reassessing Long-Term Opioid Therapy Without Abandonment

Chronic pain is one of the most consequential conditions managed in primary care, demanding compassionate, individualized treatment amid evolving evidence and persistent stigma. Misunderstandings surrounding pain, opioid therapy, and guideline implementation continue to create barriers to effective treatment, leaving many clinicians uncertain about how to balance patient-centered care with safe prescribing practices. As the 2022 CDC Clinical Practice Guideline for Prescribing Opioids reshapes pain management, clinicians need practical guidance to apply updated recommendations and the FDA Opioid Analgesic REMS Education Blueprint within individualized, multimodal treatment plans that promote safety, equity, and improved outcomes. This interactive CaseWise activity, one of four in the series, will translate these principles into realistic primary care decision making. Through a patient case and structured clinical decision points, learners will apply physiologic and biopsychosocial validated pain assessment tools, multimodal treatment strategies, and opioid risk assessment in practice. Expert faculty will compare clinical approaches, clarify evidence and guideline recommendations, and model patient counseling on opioid safety, including storage and disposal, recognition of opioid use disorder (OUD) using DSM-5 criteria, and evidence based, nonstigmatizing communication about diagnosis and treatment options across common challenges encountered in longitudinal primary care.
