Blueprint in Practice: The 2 AM Stone – Treating Severe Pain Without Creating a New Safety Problem

Providing timely, effective pain relief in emergency and urgent care settings requires clinicians to make high-stakes decisions under pressure while balancing safety and efficacy. Distinguishing acute, subacute, and chronic pain remains challenging when selecting appropriate treatment strategies, including when to integrate non-opioid, multimodal approaches alongside opioid therapy. Patients face consequences when acute pain is undertreated or managed suboptimally, including delayed recovery, reduced function, increased risk of chronic pain, prolonged opioid use, and diminished quality of life. As pain management guidance evolves, clinicians need evidence-based education to confidently assess pain, implement multimodal treatment strategies, and deliver compassionate, patient-centered care that minimizes risk while applying the 2022 CDC Clinical Practice Guideline and FDA Opioid Analgesic REMS Education Blueprint in emergency and urgent care practice. This interactive CaseWise activity, one of four in the series, will bring these principles to life through realistic emergency and urgent care decision making. Through a patient case and structured decision points, learners will apply pain assessment, multimodal treatment, opioid risk assessment, and safe prescribing strategies. Expert faculty will clarify evidence and guideline recommendations while modeling patient counseling, opioid safety, recognition of opioid use disorder using DSM-5 criteria, and nonstigmatizing communication about diagnosis and treatment options.
