Carotid Sinus Hypersensitivity

Carotid sinus hypersensitivity is a clinical syndrome defined by an exaggerated autonomic response to mechanical stimulation of the carotid sinus, typically located at the bifurcation of the common carotid artery. This hypersensitive reflex arc results in either increased vagal tone or decreased sympathetic output, leading to bradycardia, vasodilation, and hypotension. The subsequent transient cerebral hypoperfusion often presents as syncope or presyncope. Three subtypes are recognized: cardioinhibitory, vasodepressor, and mixed. Risk increases with advancing age and may be exacerbated by external factors such as tight collars, head movement, or prior neck trauma. Diagnostic evaluation requires carotid sinus massage under electrocardiographic and blood pressure monitoring to confirm a symptomatic bradycardic or hypotensive response. Timely management depends on the dominant subtype, with pacemaker implantation indicated for cardioinhibitory forms and pharmacologic or behavioral interventions for vasodepressor types. This activity enhances healthcare professionals ability to recognize, diagnose, and manage carotid sinus hypersensitivity by expanding understanding of autonomic reflex physiology, risk factors, and subtype-specific treatment strategies. Learners improve diagnostic accuracy through refined use of carotid sinus massage protocols and monitoring techniques. Emphasis on multidisciplinary care encourages collaboration among internists, cardiologists, neurologists, and geriatricians to optimize evaluation and therapeutic decision-making. Interprofessional coordination reduces the risk of recurrent syncope, fall-related injuries, and associated morbidity, ultimately improving outcomes and quality of life for patients with this underrecognized condition.
