Chiari Malformation Type 1

Clinicians encounter Chiari malformations as a spectrum of hindbrain abnormalities involving the cerebellum, brainstem, skull base, and cervical spinal cord, with Chiari 1 recognized as the most common subtype. This course reviews Chiari malformation type 1, which features caudal descent of the cerebellar tonsils of at least 5 mm and presents with symptoms ranging from tussive headaches and sleep-disordered breathing to scoliosis and focal neurological deficits, although many patients remain asymptomatic. Etiologic theories, which span genetic, hydrodynamic, and structural mechanisms, as well as associated complications such as syringomyelia, and surgical planning, are also discussed. This activity outlines the historical foundations, pathophysiology, genetic influences, clinical spectrum, and diagnostic criteria of Chiari 1 malformation, as well as the current consensus on surgical and nonsurgical management, which ranges from conservative monitoring to posterior fossa decompression when symptoms or radiographic progression warrant intervention. Participants will also gain an understanding of risk benefit considerations surrounding intervention, symptom patterns, imaging interpretation, and evidence-based treatment pathways to support more effective evaluation and longitudinal care for affected patients. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying Chiari malformation type 1, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition to improve outcomes.
