Chronic Mesenteric Ischemia

Chronic mesenteric ischemia (CMI) is a manifestation of peripheral vascular disease caused by insufficient blood supply to meet the metabolic demands of visceral organs. CMI is typically due to atherosclerotic narrowing of mesenteric vessels, most commonly the superior mesenteric artery. Risk factors include smoking, hyperlipidemia, diabetes, and hypertension. Acutely, patients often present with postprandial abdominal pain termed “abdominal angina” occurring 15 to 30 minutes after eating and lasting up to 4 hours, though additional nonspecific symptoms, including nausea, vomiting, early satiety, diarrhea, or constipation over time typically characterize CMI. This pain can lead to a fear of eating and significant weight loss. Diagnosis of CMI can be challenging secondary to various differential diagnoses with similar presentations and, therefore, involves vascular imaging with laboratory studies if indicated. Treatment depends on symptom severity. Asymptomatic patients are managed conservatively with lifestyle changes and antiplatelet therapy. Symptomatic cases require surgical revascularization. Untreated symptomatic CMI has a 5-year mortality rate approaching 100%, emphasizing the importance of timely intervention. This activity for healthcare professionals is designed to enhance the learner’s competence in recognizing CMI, understanding its pathophysiology, performing the recommended evaluation, and implementing an appropriate interprofessional management approach to improve patient outcomes.
