Cicatricial Pemphigoid

Cicatricial pemphigoid is a rare, chronic autoimmune blistering disorder that affects mucous membranes, skin, or both, and carries a significant risk of scarring and irreversible complications. Mucous membrane involvement may affect the oral, ocular, nasopharyngeal, laryngeal, esophageal, genital, and anal regions, with ocular and airway involvement posing significant risks, including blindness and respiratory compromise. Oral lesions commonly cause pain, gingival bleeding, and tooth loss, while ocular disease can progress to symblepharon, entropion, and ultimately vision loss. Diagnosis is challenging and relies on clinical suspicion supported by histopathology, direct and indirect immunofluorescence, and serologic testing. This course explores prompt recognition and early, aggressive therapy of cicatricial pemphigoid to prevent long-term complications. Commonly employed treatments include topical corticosteroids, dapsone, systemic corticosteroids, and immunosuppressive agents, as well as more advanced therapies, such as intravenous immunoglobulin and rituximab, reserved for refractory or severe disease. Supportive management for symptom control and quality of life is also reviewed. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying cicatricial pemphigoid, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition.
