Diffuse Toxic Goiter

Goiter refers to enlargement of the thyroid gland and may arise from multiple etiologies, most commonly iodine deficiency worldwide and autoimmune disease in the United States. Diffuse toxic goiter represents a hyperfunctioning, diffusely enlarged thyroid that produces excess thyroid hormones and is most often associated with Graves disease, though nonautoimmune causes also exist. This course reviews the common presentations of diffuse toxic goiter, including weight loss, heat intolerance, tremor, palpitations, and neck swelling, with some developing Graves ophthalmopathy or dermopathy. Additionally, this activity reviews the pathophysiology, clinical features, diagnostic evaluation, and evidence-based management of diffuse toxic goiter. Diagnosis relies on thyroid function testing, targeted antibody assays, and imaging when indicated. Management options include antithyroid medications, radioactive iodine therapy, or surgery, each chosen based on patient characteristics, comorbidities, and disease severity. Participants will also gain an understanding of the varied presentations, interpretation of diagnostic findings, appropriate therapy selection, and patient counseling of risks. and long-term follow-up of diffuse toxic goiter. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying diffuse toxic goiter, performing the recommended evaluation, and implementing an appropriate interprofessional approach to manage this condition, thereby reducing complications through timely, individualized care and improving outcomes.
