Heart failure with left ventricular ejection fraction (LVEF) 40% (HFmrEF and HFpEF) is an increasing public health challenge, with a lifetime risk of 24%. These subtypes are often difficult to diagnose and manage in primary care, especially in patients with obesity, type 2 diabetes, and chronic kidney disease, which contribute to poor outcomes and persistent cardiovascular risk. Although SGLT2 inhibitors and other therapies have improved care, important gaps remain in mortality reduction and symptom control. This educational initiative highlights the latest evidence, including the FINEARTS-HF trial and the role of the nonsteroidal mineralocorticoid receptor antagonist (MRA) finerenone, which has been approved to reduce worsening heart failure events and cardiovascular death in patients with LVEF 40%. Participants will also gain practical guidance on initiating and monitoring MRA therapy, identifying contraindications, and optimizing individualized care plans.
