Hypoplastic Left Heart Syndrome

Hypoplastic left heart syndrome (HLHS) is a rare but severe congenital heart defect characterized by underdevelopment of left-sided heart structures, including the left ventricle, aortic valve, mitral valve, and ascending aorta. This condition leads to critically impaired systemic circulation, necessitating a patent ductus arteriosus for survival in the immediate neonatal period. Diagnosis often occurs prenatally through fetal echocardiography, with postnatal clinical findings including cyanosis, weak pulses, and signs of low cardiac output as the ductus closes. Management requires a staged surgical approach, including the Norwood, Glenn, and Fontan procedures, to establish single-ventricle physiology compatible with life. Despite advancements in care, HLHS patients face significant risks of morbidity and mortality, including heart failure, arrhythmias, and thromboembolic events, with long-term survival dependent on specialized medical and surgical interventions. This course provides clinicians with a comprehensive understanding of HLHS, from its pathophysiology and diagnostic approach to the intricacies of surgical management and long-term care. Participants gain insights into optimizing perioperative hemodynamics, managing complications of staged palliation, and addressing long-term issues such as Fontan physiology failure and associated comorbidities. Additionally, the course highlights the importance of interdisciplinary collaboration and patient-family education in improving outcomes. Clinicians emerge with enhanced clinical competence and confidence to care for patients with HLHS across the lifespan, contributing to improved survival rates and quality of life for this complex population.
