Interprofessional Care of Patients with Pulmonary Arterial Hypertension

A chronic, progressive disease that causes ongoing pulmonary vascular remodeling, pulmonary arterial hypertension (PAH) causes substantial patient burden beginning with significant diagnostic delay and declining physical function. Combination therapy with targeted agents is now recommended beginning at the time of diagnosis. Moreover, as recommended in current guidelines, the complex nature of PAH requires a multifaceted, holistic, and collaborative team-based approach to care. This activity highlights key concepts in managing patients with PAH, provides an overview of the principles of interprofessional collaboration, and describes the benefits and roles of members of an interprofessional, multidisciplinary care team for PAH.
