Kidney Health Coach: Cardiovascular-Kidney-Metabolic (CKM) is designed to educate healthcare professionals about CKM syndrome, including its definition, key components, risk factors, and strategies for prevention and management. The program focuses on improving the quality of care for individuals at risk for or living with CKM syndrome by addressing gaps in awareness, education, and support. Through community-based education and outreach, Kidney Health Coach: CKM aims to increase understanding of CKM syndrome, promote early identification of risk factors, and empower individuals to take proactive steps to prevent, manage, and improve their overall cardiovascular, kidney, and metabolic health.
