Lateral Medullary Syndrome (Wallenberg Syndrome)

Lateral medullary syndrome is a posterior circulation stroke syndrome caused most often by ischemic injury to the dorsolateral medulla from vertebral artery or posterior inferior cerebellar artery involvement. Clinical manifestations may include vertigo, nystagmus, ataxia, dysphagia, dysphonia, ipsilateral facial sensory loss, contralateral body pain and temperature loss, Horner syndrome, hiccups, and respiratory or autonomic complications. A practice gap persists in clinicians ability to distinguish lateral medullary syndrome from peripheral vestibular disorders, recognize partial or atypical presentations, select appropriate neuroimaging, apply acute ischemic stroke treatment pathways, and coordinate rehabilitation and secondary prevention. This activity addresses that gap by reviewing neuroanatomy, clinical findings, diagnostic evaluation, imaging strategies, reperfusion considerations, complication prevention, and interprofessional rehabilitation. Participants are expected to improve diagnostic accuracy, timely stroke recognition, treatment selection, complication prevention, and coordinated patient-centered recovery.
