Liddle Syndrome (Pseudohyperaldosteronism)

Liddle syndrome, also known as pseudohyperaldosteronism, is a rare hereditary cause of early-onset, resistant hypertension resulting from a gain-of-function mutation in the epithelial sodium channel of the distal nephron. This mutation promotes excessive sodium reabsorption and potassium excretion, leading to hypertension, hypokalemia, and metabolic alkalosis. The syndrome follows an autosomal dominant inheritance pattern characterized by low plasma renin and aldosterone levels, distinguishing it from other mineralocorticoid excess states. Phenotypic variability and incomplete penetrance can obscure recognition, often resulting in delayed diagnosis or misclassification as essential hypertension or primary hyperaldosteronism. Without timely intervention, patients face increased risks of cardiovascular and renal complications, including heart failure, stroke, and chronic kidney disease. Early identification and targeted treatment with epithelial sodium channel inhibitors such as amiloride or triamterene are essential for controlling blood pressure and preventing disease progression.
