Macrocytic Anemia

Macrocytic anemia is a condition that occurs secondary to various underlying etiologies, characterized by an increased mean corpuscular volume (MCV >100 fL) in conjunction with anemia, defined by low hemoglobin or hematocrit levels. Macrocytic anemia is subcategorized into megaloblastic and nonmegaloblastic forms. Megaloblastic anemia, often caused by folate or vitamin B12 deficiencies, results from impaired DNA synthesis and is marked by hypersegmented neutrophils. Nonmegaloblastic anemia, which lacks hypersegmented neutrophils, can result from alcohol use, hereditary spherocytosis, hypothyroidism, liver disease, or conditions causing increased reticulocyte production. Some cases of macrocytosis are normal variants, while others result from laboratory artifacts. This course explores the clinical considerations surrounding this condition, including the diagnostic challenges secondary to varying symptomatic presentations, wide-ranging differential diagnoses, and targeted management approaches. Clinical presentation and treatment differ based on the underlying etiology. Therefore, an in-depth understanding of macrocytic anemia’s pathophysiology and resulting manifestations is essential.
