Migraine Surgical Interventions

Migraine is a prevalent primary headache disorder marked by recurrent episodes of moderate to severe pain, often accompanied by nausea, vomiting, photophobia, or phonophobia. Some patients experience an aura due to cortical spreading depression. The pathophysiology involves trigeminovascular activation, neurogenic inflammation, and cortical hyperexcitability. Standard management includes pharmacologic and behavioral therapies, yet a subset of patients develop chronic, drug-refractory migraine. For these individuals, advanced options such as botulinum toxin injections, peripheral nerve neurolysis or neurectomy, and neuromodulation with occipital nerve stimulation may be considered. Selection of appropriate surgical candidates requires careful clinical evaluation, recognition of trigger sites, and assessment of comorbidities to balance benefit with risks such as muscle weakness, neuropathic pain, or device-related complications. This course strengthens clinicians’ knowledge and competence in the evaluation and management of refractory migraine. Learners explore the mechanisms, techniques, and evidence supporting advanced interventions, and they develop strategies for safe patient selection and complication prevention. The curriculum highlights interprofessional collaboration among neurologists, surgeons, psychologists, psychiatrists, pharmacists, and specialty-trained nurses. Each professional brings unique expertise, encompassing medication optimization, behavioral health, surgical planning, and patient education. Collaborative care improves diagnostic accuracy, procedural safety, and adherence, ultimately enhancing patient outcomes and quality of life.
