Nonsteroidal Anti-Inflammatory Drugs Toxicity

Nonsteroidal anti-inflammatory drugs (NSAIDs) are a diverse class of medications that, despite chemical differences, share common therapeutic and adverse effects. They primarily alleviate inflammation, pain, and fever by inhibiting cyclooxygenase (COX) enzymes, which are crucial in the biosynthesis of prostanoids. The two primary COX isozymes, COX-1 and COX-2, convert arachidonic acid into prostaglandins, prostacyclin, and thromboxane molecules involved in various physiological processes, including inflammation, regulation of smooth muscle tone, and thrombosis. COX-1 is constitutively expressed and plays a crucial role in maintaining physiological functions, such as gastric mucosal protection. In contrast, COX-2 is inducible, and its expression increases during inflammatory states. While NSAIDs’ inhibition of these enzymes underlies their therapeutic benefits, it also contributes to potential toxicities. This activity provides an updated review of the etiology, presentation, evaluation, and management of NSAID toxicity, highlighting the role of the interprofessional team in managing this condition.
