Older adults represent one of the fastest-growing populations affected by obesity, yet obesity management in this group presents unique challenges that extend beyond weight loss alone. Treatment goals should focus on improving complications of excess adiposity, preserving functional status, enhancing quality of life, and minimizing the loss of muscle and bone mass that may accompany intentional weight loss. The Medicare Bridge Program has created a new opportunity for eligible Medicare beneficiaries to access anti-obesity medications, but clinicians must navigate complex questions regarding patient selection, sarcopenia and frailty, medication adverse effects, polypharmacy, deprescribing, and long-term treatment sustainability when coverage changes. This session will begin with a practical overview of the Medicare Bridge Program, including eligibility requirements and implementation considerations, followed by two real-world patient cases illustrating common clinical dilemmas in obesity management among older adults.
