Paget-Schroetter Syndrome

Paget-Schroetter syndrome is a primary upper extremity deep vein thrombosis characterized by effort-induced axillary or subclavian vein thrombosis, typically occurring in young, healthy individuals following repetitive upper extremity exertion. The condition is considered the venous form of thoracic outlet syndrome, including neurogenic and arterial subtypes. Pathophysiology involves mechanical compression at the thoracic outlet often related to anatomical anomalies such as cervical ribs or hypertrophied scalene muscles combined with endothelial injury and venous stasis, fulfilling the Virchow triad. Clinical presentation includes sudden-onset arm swelling, pain, heaviness, and sometimes cyanosis. Diagnosis is guided by clinical suspicion and confirmed by duplex ultrasonography, venography, or cross-sectional imaging. Prompt management with thrombolysis, anticoagulation, and surgical decompression improves long-term outcomes and reduces complications such as recurrence or postthrombotic syndrome. This activity enhances clinician competence in recognizing, diagnosing, and treating Paget-Schroetter syndrome. Participants gain a deeper understanding of the anatomic and mechanical contributors to venous obstruction and current diagnostic strategies and interventional therapies. Emphasis is placed on early intervention and selecting appropriate candidates for surgical decompression. Improved interdisciplinary coordination optimizes patient outcomes, limits recurrence, and supports return to normal function in active individuals.
