Parkinson Disease

Parkinson disease (PD) is a progressive neurodegenerative disorder characterized by the degeneration of dopaminergic neurons in the substantia nigra and the accumulation of alpha-synuclein within Lewy bodies. The condition affects approximately 1% of individuals older than 60 years and represents the fastest-growing neurodegenerative disorder worldwide. Clinical presentation most commonly begins with asymmetric motor features, including bradykinesia, resting tremor, rigidity, and postural instability. Nonmotor symptoms such as constipation, sleep disturbance, depression, cognitive decline, and autonomic dysfunction often precede or accompany motor findings, contributing significantly to morbidity. By the time of diagnosis, more than half of dopaminergic neurons are already lost, and evidence suggests that pathological changes may begin a decade or more before motor manifestations. Although most cases are idiopathic, approximately 10% of patients have a genetic cause. Diagnosis is primarily clinical, supported by dopamine transporter imaging when uncertainty exists. Management focuses on symptom control through pharmacologic therapy, rehabilitation strategies, and in select cases, surgical intervention, such as deep-brain stimulation. This activity enhances clinician competence by strengthening recognition of early motor and nonmotor features, improving diagnostic accuracy, and facilitating differentiation of PD from other Parkinsonian syndromes, such as essential tremor or atypical Parkinsonism. Participants gain the ability to optimize pharmacologic and nonpharmacologic management strategies, tailor therapy to disease stage and patient needs, and evaluate candidacy for advanced therapies, including surgical options.
