Pediatrics: Risk Management Issues

Pediatrics presents unique patient safety and risk management challenges across the continuum of care, from prenatal and neonatal periods through adolescence and young adulthood. Although pediatricians face malpractice claims less frequently than many other specialties, pediatric claims are associated with disproportionately high indemnity payments, often driven by catastrophic outcomes, lifelong disability, and the emotional impact of harm to children. Risk varies by age group, with neonates and infants representing particularly vulnerable populations, though significant claims occur across all pediatric age ranges. This course examines risk management and patient safety issues in the general practice of pediatrics, focusing on pediatrics as a primary care discipline. Key topics include bilirubin encephalopathy, delayed diagnosis, child abuse recognition and reporting, consent and decision-making related to neonatal resuscitation and viability, and medical screening of newborns under EMTALA. Through case-based analysis, the course highlights common clinical, diagnostic, and systems-based failures that contribute to adverse outcomes and liability exposure. After completing this activity, practitioners and the healthcare team should be able to: implement strategies for the prevention and management of bilirubin encephalopathy in neonates; evaluate diagnostic delays in pediatric malignancies and apply measures to improve early detection; improve the detection and reporting of suspected child abuse in compliance with mandatory reporting laws; and assess the importance of medical screening for newborns under EMTALA and apply legal and ethical principles in determining infant viability and ensuring appropriate care.
