Psoas Syndrome

Psoas syndrome is characterized by pain and dysfunction of the psoas or iliopsoas muscle group due to irritation, inflammation, or injury. This condition frequently affects individuals engaged in repetitive hip flexion, such as runners, dancers, and high jumpers, but can also occur in nonathletes due to prolonged sitting, poor posture, or spinal pathology. Less common causes, including psoas abscess or retroperitoneal malignancy, should be considered in atypical or refractory presentations. Mechanical strain may lead to spasm, inflammation, or fibrosis. Tendon snapping over bony structures can cause anterior or internal snapping hip syndrome (coxa saltans or dancer s hip ), which may coexist with tendonitis or follow total hip arthroplasty. Patients typically report deep anterior hip or groin pain worsened by extension or resisted flexion, sometimes radiating to the thigh or lumbar region. Audible or visible snapping may be present, particularly in adolescent athletes during growth spurts. Diagnosis is primarily clinical, aided by tests such as the Thomas test or resisted straight-leg raise. Imaging helps rule out abscess or malignancy. Treatment begins with rest, NSAIDs, physical therapy, and activity modification. Corticosteroid injections may be used in refractory cases, with surgical release reserved for persistent symptoms. This activity focuses on evaluating and managing psoas syndrome, covering the best diagnostic and therapeutic practices. This activity also broadens clinicians understanding of the condition s etiology, pathophysiology, and clinical presentation. This activity highlights the importance of interprofessional collaboration among healthcare providers to work effectively within a care team to improve patient outcomes.
