Pulmonary Interstitial Emphysema

Pulmonary interstitial emphysema (PIE) is a rare but clinically significant lung condition that predominantly affects premature infants, although it can also occur in adults. Infants with low birth weight and prematurity are at increased risk for PIE, underscoring the need for early recognition and timely intervention. PIE is characterized by alveolar and airway over-distention, resulting in air leakage into the interstitial and bronchovascular spaces, which disrupts normal lung architecture and may lead to severe complications such as pneumothorax or pneumopericardium. PIE often occurs in neonates with respiratory distress syndrome and is typically diagnosed through radiological imaging, which reveals linear or cystic air-filled lucencies in the lung parenchyma. Clinicians should adopt a comprehensive diagnostic approach that includes exclusionary criteria, detailed physical examinations, and targeted imaging to improve diagnostic accuracy. Management strategies include the use of prophylactic surfactant therapy, noninvasive ventilation techniques, and, in refractory cases, mechanical ventilation or surgical options such as lobectomy.
