Sea Snake Toxicity

Sea snakes are highly venomous, fully aquatic elapids found in tropical and subtropical marine waters, particularly in the Indo-Pacific region. Human envenomation is uncommon but can occur among fishermen, divers, and coastal laborers who inadvertently provoke or handle these serpents. Although not all bites result in venom injection, clinically significant envenomation can lead to severe systemic toxicity. Sea snake venom contains potent neurotoxins and myotoxins that disrupt neuromuscular transmission and cause rapid, diffuse muscle breakdown, potentially leading to paralysis, rhabdomyolysis, myoglobinuria, acute kidney injury, and death. Symptoms may be delayed for hours and include myalgia, trismus, ptosis, dysphagia, and flaccid paralysis. Diagnosis is clinical, based on exposure history, early neuromuscular signs, and laboratory evidence of muscle injury. Management includes prompt immobilization, supportive care, aggressive hydration, and timely administration of sea snake-specific antivenom in symptomatic cases. Possible complications include respiratory failure, acute tubular necrosis, and hyperkalemia. The prognosis is generally favorable with rapid recognition and treatment.
