Sonographic Evaluation of Amniotic Fluid

Sonographic evaluation of the amniotic fluid index (AFI) is a standardized and noninvasive method used during pregnancy to assess amniotic fluid volume, which is critical for fetal development and well-being. Amniotic fluid provides a protective environment, facilitates movement, supports temperature regulation, and contributes to the development of fetal organs such as the lungs. Additionally, the volume of amniotic fluid, produced and processed primarily by the fetus, reflects placental function. Measuring the AFI involves dividing the uterus into 4 quadrants and measuring the deepest vertical pocket of fluid in each quadrant, with these measurements combined to calculate the total AFI. A normal AFI ranges from 5 to 24 cm in singleton pregnancies, with deviations indicating oligohydramnios (low fluid) or polyhydramnios (excess fluid). An alternative evaluation method is the maximal vertical pocket (MVP) measurement, which is particularly useful in cases of multiple gestations or when AFI results are unclear. Oligohydramnios, often caused by placental insufficiency or membrane rupture, can lead to complications such as pulmonary hypoplasia, fetal distress, and increased perinatal morbidity. Conversely, polyhydramnios, associated with conditions like maternal diabetes or fetal swallowing impairments, may result in preterm labor, macrosomia, or respiratory discomfort for the mother. Regular sonographic evaluation of amniotic fluid is integral to monitoring high-risk pregnancies, guiding clinical interventions, and ensuring optimal outcomes for both mother and fetus. This activity for healthcare professionals is designed to enhance the learner’s competence in appropriately performing sonographic techniques to evaluate amniotic fluid volume, interpreting evaluation findings, and implementing an appropriate interprofessional management approach to improve patient outcomes.
