Subjective Refraction Technique: Astigmatic Dial

Astigmatism is a refractive condition that most commonly results from unequal curvature of the cornea, producing unequal dioptric power between the principal meridians of the eye. This ocular disorder is usually congenital but may change in magnitude and axis with age. Although automated refractive devices and corneal topography have significantly improved the diagnosis and characterization of astigmatism, subjective refinement techniques remain essential for determining an individual’s final functional refractive endpoint. Astigmatic dial testing continues to provide clinically useful information for cylindrical axis localization, assessment of patient-perceived responses, and refinement of refractive correction. Astigmatic dials include the fan, clock, and Lancaster dials and may be stationary or rotary. This activity reviews the principles of astigmatic dial testing and describes the procedures for its clinical use. Emphasis is placed on selecting candidates, executing the technique, and interpreting the findings correctly to optimize astigmatism management within an interprofessional care setting.
