Trauma: Traumatic Brain Injury

Head injury represents a high-risk area for healthcare practitioners, with more than 750,000 emergency department visits for intracranial head injury each year. Medical errors and adverse outcomes related to traumatic brain injury (TBI) can result in permanent disability or death. Diagnosis is often challenging, and uncertainty remains regarding optimal management strategies. To improve outcomes and reduce risk, clinicians must apply a systematic, risk-managed approach to the evaluation and management of patients with head injury. This activity provides practical guidance on head injury risk assessment and documentation, reviews key definitions and clinical recommendations, and outlines essential elements of the neurologic examination. It also examines patient populations at highest risk for TBI, delayed intracranial injury, and considerations related to safe discharge of head-injured patients. Case-based discussions highlight common causes of missed or delayed diagnoses, delayed or inappropriate management, and medicolegal exposure. After completing this activity, practitioners and members of the healthcare team should be able to: evaluate patients with head trauma with an awareness of the common reasons for the failure to diagnose TBI to improve diagnostic accuracy, enhance documentation, and reduce medicolegal risk; document pertinent positives and negatives in the assessment of head-injured patients to support clinical reasoning, demonstrate a thorough differential diagnosis, and strengthen medicolegal defensibility; and apply evidence-based guidelines for CT imaging in pediatric head trauma to ensure appropriate use of imaging and improve the detection of TBI.
