This activity has been developed for surgeons who work in clinical practice and have difficulty staying up to date on the myriad of rapid advancements and innovation in medicine. There are numerous publications detailing the fact that it has essentially become impossible for physicians to keep up with the rapid advancements in medical innovation. For example, in an interesting study back in 2004 by Alper et al. it was estimated that it would take 627.5 hours per month to evaluate the 7,287 articles published monthly in high-quality primary care journals. In 2005, a systematic review in the Annals of Internal Medicine made headlines when it reported a correlation between increasing years in practice and decreasing quality of care. This article suggested that the longer physicians have been in practice, the less likely they were to adhere to updated clinical guidelines, leading to worse patient outcomes. A study in 2011 estimated that the doubling time of medical knowledge in 1950 was 50 years; in 1980, 7 years; and in 2010, 3.5 years. In 2020 it is projected to be just 73 days. This report suggests that the problem is becoming worse in light of what has become an exponential explosion of medical innovation. This activity is designed to help address this problem; while it is impossible for any physician to read and absorb every published research article relevant to their field, this activity will aide physicians in selecting the articles that are most relevant to their specific area of practice by utilizing machine learning which integrates learned preferences of the physician with a database of physicians in similar practice along with specifics such as H-index and SJR of the article. At the same time, we will assess the outcomes of the activity in both a quantitative (pre-test and post-test) and qualitative (CME evaluation) means for each participant and this data will also be incorporated into the recommendation algorithm to improve future learning for the participant. We expect that physicians who complete this activity will develop a more complete understanding of at least one facet of trending evidence-based medical innovation which is relevant to their practice as well as apply the information learned to increase their competence and performance when It comes to the care of their patients.
