Tuberculous Spondylitis (Pott Disease)

Tuberculous (TB) spondylitis, also known as Pott s disease, represents a severe extrapulmonary manifestation of TB and remains a significant cause of morbidity worldwide. Although early disease may respond to conservative anti-TB therapy, delayed diagnosis frequently leads to progressive vertebral destruction, spinal instability, kyphotic deformity, abscess formation, and neurologic compromise. The thoracolumbar spine is most commonly affected, with children and individuals from high-incidence regions or immigrant populations at particular risk. This course reviews the diagnosis of TB spondylitis, which often proves challenging because symptoms are insidious, constitutional features are uncommon, and microbiologic confirmation may be negative, necessitating reliance on imaging, histology, and clinical suspicion, and its management, which requires prolonged chemotherapy and, in many cases, surgical intervention to decompress neural elements and restore spinal stability. This activity outlines the epidemiology, pathophysiology, clinical presentation, imaging characteristics, and management strategies for TB spondylitis. Participants will also gain an understanding of diagnostic reasoning and the interpretation of radiologic findings, as well as the indications for medical versus surgical treatment. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying TB spondylitis, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition to optimize patient outcomes and prevent long-term complications.
